Pressure Urticaria

Delayed Hives and Swelling From Sustained Pressure

Pressure urticaria causes deep, often painful swelling or welts at sites of sustained pressure. Unlike dermatographism, the reaction is delayed, typically appearing 30 minutes to 12 hours after the pressure, and can last up to a day or more.

Symptoms

Swelling that feels tender, burning or itchy appears where straps, belts, tight shoes or hard seats pressed. Hands (after tool use), feet (after walking) and buttocks are common sites. Some people also feel flu-like tiredness or joint aches.

Common Triggers

  • Bra straps, waistbands and backpack straps
  • Tight footwear or long walks
  • Manual work with tools
  • Prolonged sitting on hard surfaces

Diagnosis and Related Conditions

Doctors may apply a weighted strap to the thigh or back and examine the skin hours later. Pressure urticaria often occurs together with chronic spontaneous urticaria. Compare with immediate dermatographic urticaria.

Management

Reduce pressure with wider straps, padded seats and well-fitting shoes. Antihistamines tend to help less for this type, so your doctor may discuss other medicines for stubborn cases. Learn about the range of hives types.

When to Call 911

Do not wait it out: hives with shortness of breath, noisy breathing, a hoarse voice, swelling of the tongue or throat, dizziness, a racing pulse, or repeated vomiting are signs of a possible severe allergic reaction (anaphylaxis). Call 911 immediately.

When to See a Doctor

See a doctor if your symptoms last more than a few days, keep coming back, cover large areas, or interfere with sleep or daily life. The Hives Treatment Center in San Francisco, founded by Dr. Tiffany Young, is a dermatology practice dedicated to hives and related conditions, and is accepting new patients.

Frequently Asked Questions About Pressure Urticaria

How long does pressure urticaria last?

Often several years, though symptoms vary.

Why is it painful?

Deeper swelling presses on nerves, so it often hurts more than it itches.

Does it affect work?

It can, especially with manual jobs or prolonged standing, so adjusting equipment helps.
